St. Vincent's is the first hospital in Fairfield County to be integrated into the Hartford HealthCare network, and is the system's second-largest hospital.OverviewHHC is seeking a dedicated and caring Patient Assessment Instrument Coordinator (IRF-PAI) to be part of an experienced team that brings decades of experience in providing the most innovative care. This position coordinates the data collection on the Inpatient Rehabilitation Facility Patient Assessment Instrument (IRF-PAI), ensures the presence of supporting documentation, and provides staff education to ensure compliance with CMS guidelines. The IRF-PAI Coordinator organizes all efforts for immediate issue resolution concerning FIM and Quality scoring. The IRF-PAI Coordinator conducts ongoing review of IRF-PAI related data, and actively collaborates with coding, nursing, therapy and case management to ensure accurate and timely IRF-PAI completion.
