St. Vincent's Medical Center in Bridgeport has more than 3,200
employees. It includes a 473-bed community teaching hospital, a
76-bed inpatient psychiatric facility in Westport, a large
multispecialty provider group, and special needs services for
adults and children. St. Vincent's is the first hospital in
Fairfield County to be integrated into the Hartford HealthCare
network, and is the system's second-largest hospital. Overview HHC
is seeking a dedicated and caring Patient Assessment Instrument
Coordinator (IRF-PAI) to be part of an experienced team that brings
decades of experience in providing the most innovative care. This
position coordinates the data collection on the Inpatient
Rehabilitation Facility Patient Assessment Instrument (IRF-PAI),
ensures the presence of supporting documentation, and provides
staff education to ensure compliance with CMS guidelines. The
IRF-PAI Coordinator organizes all efforts for immediate issue
resolution concerning FIM and Quality scoring. The IRF-PAI
Coordinator conducts ongoing review of IRF-PAI related data, and
actively collaborates with coding, nursing, therapy and case
management to ensure accurate and timely IRF-PAI completion. The
IRF-PAI Coordinator also oversees the development of tools that
assist in compliance and quality reporting for the Inpatient Rehab
Unit (IRU).Qualifications: Current CT licensure in nursing or
allied health profession required (PT/OT/SLP/PTA/COTA). Associate's
or Bachelor's degree required Minimum 2 years' experience in
rehabilitation or closely related field required, inpatient
rehabilitation specific experience highly preferred Experience with
FIMs and IRF-PPS preferred Knowledge, Skills and Ability
