Primary Purpose:

The Homeless Prevention Program (HPP) is a statewide initiative designed to assist households (individuals and families) who are at high-risk of becoming homeless. The Homelessness Prevention Case Worker position is located in New Haven. The Homelessness Prevention Case Worker will conduct intakes with clients and coordinates services among regional partners with the goal of ensuring stable housing for clients.

Essential Functions:

* Provide comprehensive case management to households at risk of homelessness in the Greater New Haven region * Assist clients with stabilizing housing, including landlord/tenant mediation, linkage to housing court legal representation, affordable housing search, tenancy skill building and responsibilities, fiscal preparedness for independent rent payment, and housing maintenance. * Assist clients with identifying unaddressed needs related to benefits, employment and income, education, housing, medical conditions, mental health, substance use, family and/or community based supports, and access to mainstream services. Refer and support engagement in appropriate services to meet the family's needs. * Utilize the statewide HMIS database to document services and client outcomes * Conduct needs assessment of the household to determine a plan to maintain stable housing * Coordinate services between HPP program components including mediation services and legal services * Maintain on-going client files, including assessments, service plans, correspondence, and case notes. * Participate in statewide and regional HPP meetings as needed * Other duties as assigned to assure successful operations

Qualifications:

* Bachelor's degree AND two years of experience * Bilingual in Spanish, both written and oral required * Valid CT Driver's License, proof of insurance and access to reliable transportation * Proficient in Microsoft Outlook, Word, Excel and Power Point * High level of organizational skills and ability to prioritize and manage multiple projects simultaneously * Highly motivated, creative with excellent interpersonal skills * Excellent written and verbal communication skills * Ability and desire to work as part of team; identify with and support the mission of New * A commitment to the values of New Reach: Professionalism, Integrity, Compassion and Initiative.

Preferred Qualifications

* Experience in housing and housing law * Knowledge of leasing procedures and fair housing laws.
