As a clinical liaison, your job duties include performing patient intake evaluations, admitting patients to the Skilled Rehab unit, tracking patient records and data, processing health insurance information, and collaborating with other staff to improve processes.

Duties and Responsibilities

* Respond to patient referrals * Conduct pre-screening of potential patients to determine their eligibility for admission * Review medical records * Process payer information and ensure requirements are met * Coordinate patient discharges and transfers * Promote our services to qualified patients and their family members or caregivers

Requirements and Qualifications

* Current RN state license * Clinical or case management experience * Analytical skills

We have a wonderful opportunity for a qualified Clinical Professional who has experience in a Skilled Nursing Setting. The community Liaison will be our community outreach coordinator and will plan and carry out our community relations program. This may include conduction personal visits to prospective residents in their homes or in the hospital; visiting other communities and hospitals, talking to nurses and doctors for prospective referrals and following up on all leads. You will follow through with the entire referral process from beginning to end, ensuring compliance with State and Federal laws and Watermark policies. You will work closely with residents, families, Professional entities, Clinical resources as well management team members.

Qualifications include:

* Outreach or Community Relations experience preferred

* Outstanding interpersonal skills with a high level of energy and enthusiasm

* Excellent communication skills

* Excellent organizational skills and ability to work under pressure and meet deadlines

* Excellent computer skills

* Knowledge of State and Federal regulations

Job Requirements / Responsibilities:

* Develop referrals from multiple sources that include hospital, skilled nursing facilities, physician offices, Community based organizations etc.

* Review clinical documentation and communicate the requirements of each referral to include full assessment and follow up review.

* 1-3 years experience in healthcare marketing and outreach, case management.

* Establish marketing connections and develop professional relationships with doctors and hospitals

* Excellent interpersonal, organizational, communications, and presentations skills

* Able to work some weekends and holidays

* Excellent work ethic

* Great assessment skills
