Job Description:
Sigma Health Rehab is actively seeking a Full-time Physical
Therapist (PT) to work in a Skilled Nursing Facility. Who we are:

Responsibilities Include: - Evaluate residents’ conditions &
develop appropriate physical rehabilitation plans - Relieve
resident’s pain using exercises, hands-on therapy & stretching -
Help residents through their tasks and monitor their performance -
Supervise the activities of physical therapy assistants - Maintain
resident confidence & protects facility by keeping information
confidential - Complies with federal, state, and local legal and
professional requirements - Maintains a cooperative relationship
among healthcare team members & contributes to team effort PT
Requirements Include: - Graduate of an accredited Physical Therapy
Program - Current New York State PT License - Experience in a
skilled nursing or geriatric care setting is preferred, but new
graduates with relevant fieldwork experience are encouraged to
apply - Current evidence-based practices consistent with the
standards and recommendations of the American Physical Therapy
