Job Description Halcyon Nurse Staffing is seeking a local contract Home Health Physical Therapist for a local contract job in Wall Township, New Jersey. Job Description & Requirements - Specialty: Physical Therapist - Discipline: Therapy - Duration: 13 weeks - 40 hours per week - Shift: 8 hours, days - Employment Type: Local Contract Title: Physical Therapist Facility: Hackensack Meridian Health Location: 1340 Campus Parkway, Wall Township, NJ 07753 Duration: 13 weeks Shift: Day shift Target start date: ASAP Local rates: 40 hours per week (Within 50 miles from the facility) $88/hour (40 Regular hours) $3,520 gross per week Healthcare Benefits: Medical, Dental, and Vision. Our base plans are all free and covered by the company. Required Skills, Experience, Education & Certifications: Required Education: Bachelor’s Degree in Physical Therapy (PT) Required Years of Experience: minimum 1 year Required Licenses & Certifications: Certified Home Care experience (Part A) with OASIS knowledge and current usage. EPIC preferred. Registered Physical Therapist. BLS must be AHA. Note: We need a PT who already has certified (Medicare) home care experience and knows how to complete an OASIS assessment.
