provides compassionate care and assists in managing and delivering
patient care plans in a home health and hospice setting. This role
requires CNA certification, hospice experience, and the ability to
drive to patient locations while using mobile technology for

Nursing Assistants with: • Certified Nursing Assistant
Certification in the state you work • High School diploma or
equivalent preferred • Hospice experience preferred • Current
driver’s license and ability to spend ~20% of your day driving
to/from patient locations • The skills needed to self-manage your
time and schedule • Demonstrated experience with tablets, mobile
