contract nurse RN Ambulatory Care Supervisor for a local contract
nursing job in Brooklyn, New York. Job Description & Requirements -
Specialty: Ambulatory Care - Discipline: RN - Duration: 13 weeks -
40 hours per week - Shift: 8 hours, days - Employment Type: Local
Contract WCS Healthcare is currently looking for an Care Manager RN
to work with a large social services agency in Brooklyn working
with the geriatric population. This position offers a M-F 8:30A -
5P schedule, supervising Community Health RNs and assisting with
case management. Job Function: /n - Day Shift. /n - 8:30am - 5pm.
/n - Review and ensure effective development and execution of
personalized care plans that result in improved satisfaction &
outcomes for all participants. /n - Facilitate and complete the
process of Interdisciplinary Team (IDT) meetings, Service Delivery
Requests (SDR) and other meetings. This includes completing the
attestations, as needed. Support and collaborate with the Virtual
IDT as needed /n - Serve as a manager on call during
non-operational hours of the physical sites and responsible for
follow ups including follow up documentation and initiating
grievances on participants/caregivers’ behalf in timely manner. /n
/n Job Requirements: /n - 1 year of Supervisory or Managerial
experience. /n - Registered Nurse in New York. /n - BLS. /n
