Nurse (RN) utilize their extensive education and training to
provide personalized care to our patients in a rewarding
environment that promotes autonomy. Job Description · Support our
patients, families, and loved ones, as they go through their end of
life journey · Perform a comprehensive assessment to establish
goals for symptom management and end of life care with effective
case management strategies · Be a member of an active
interdisciplinary team that addresses patients’ needs holistically
· Develop and maintain a therapeutic relationship by utilizing
effective communication with patients and their families. · Utilize
state-of-the-art technology to document skilled services provided.
Qualifications · Registered Nurse with Valid CT RN license · One
year of RN experience; Homecare or Hospice experience preferred. ·
Valid driver’s license and reliable transportation required. ·
