Description Job Schedule: Full Time Standard Hours: 40 Job Shift: Shift 1 Shift Details: Monday thru Friday 8am to 4:30pm with one weekend per month and 1 major and 2 minor holidays per year Work where every moment matters. Perform comprehensive assessments to establish an individualized care plan. Facilitate communication between the patient and the interdisciplinary team. Develop and maintain a therapeutic relationship by providing education and promoting self-advocacy and independence. Utilize state-of-the-art technology to document skilled services provided. Qualifications Qualifications Registered Nurse with Valid CT RN license One year of RN experience; Homecare experience preferred. Valid driver---s license and reliable transportation required.
